This prestigious organization, comprised of residents of Arizona, Arkansas, Louisiana, New Mexico, Oklahoma, Texas, Colorado, Wyoming and Montana, exists to advance the science of oral and maxillofacial surgery in the fields of education, research, professional service and public relations.

Please complete the membership application below and return it to our main office along with along with the $35.00 application fee (made payable to SWSOMS). Applications may be completed online and submitted with electronic signature via email.  Upon receipt, your application for membership will be forwarded to the Chair of the Membership Committee for verification of credentials.

The next annual meeting of the Southwest Society of Oral and Maxillofacial will be held in Charleston, South Carolina at The Charleston Place Hotel (www.charlestonplace.com), April 22 – 24, 2010. Your application for membership will be included in the candidates presented for membership during our annual meeting provided we are in receipt of such application by January 16, 2010 and we are able to confirm your credentials.
